Laboratory Test Forms with Barcode Label Options

  • Applied Barcode Label: Label is glued to the front of the form on a piggyback carrier but, are removable.
  • Integrated Label: Label is part of the form, glued to the back of the sheet on a piggyback carrier and the sheet is die cut so the label shows through. This makes the label removable.
  • The carrier for the label can be an NCR sheet so a copy of what you print will stay on the carrier.
